Who We Are
Humanitas Hospice, operated by Humanitas Health Services Inc. (“Humanitas,” “we,” “us,” or “our”), provides physician-led, family-centered hospice and palliative care throughout our service area. This Privacy Policy explains how we collect, use, and protect information when you visit humanitashospice.com (the “Website”), and how we handle Protected Health Information (“PHI”) in accordance with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).
Scope of This Policy
This Policy covers two distinct kinds of information:
- Website Information — information collected through your general use of this Website, such as when you fill out a contact form, complete our online care assessment, or browse our pages.
- Protected Health Information (PHI) — individually identifiable health information we create or receive while providing hospice and palliative care to a patient. PHI is governed by our HIPAA Notice of Privacy Practices and by federal and state law, described further below.
Please do not submit medical history, diagnosis information, medication details, insurance ID numbers, or other sensitive health information through our general website contact forms or by standard email. These channels are not intended for the transmission of PHI. If you need to share information about a patient’s care, please call us directly at (215) 918-7830 or use the secure channel your Humanitas care team has provided to you.
Information We Collect
We may collect the following categories of information when you use our Website:
- Contact information you provide, such as your name, phone number, email address, and relationship to a patient, when you submit our contact form, referral form, or online care assessment.
- Responses to our online care assessment, which may include general information about symptoms, caregiving circumstances, insurance type, and timeline for care, provided voluntarily to help us understand whether hospice or palliative care may be appropriate.
- Technical information such as your IP address, browser type, device type, and pages visited, collected automatically through cookies and similar technologies.
- Call information. Calls to phone numbers listed on this Website may be tracked, and in some cases recorded, for quality assurance and marketing analytics purposes, as described further below.
How We Use Your Information
We use the information described above to:
- Respond to your inquiries and connect you with a member of our care team;
- Assess whether hospice or palliative care services may be appropriate for you or your loved one;
- Improve our Website and understand how visitors use it;
- Communicate with you about our services, unless you have opted out;
- Comply with applicable law and protect the rights, safety, and property of Humanitas Hospice, our patients, and the public.
We do not sell your personal information.
HIPAA and Protected Health Information
Humanitas Hospice is a “covered entity” under HIPAA. Once you or your loved one become a patient, the health information we create and maintain as part of care is protected under HIPAA and is governed by our Notice of Privacy Practices, which describes in detail:
- How we may use and disclose PHI for treatment, payment, and healthcare operations;
- The limited circumstances in which we may use or disclose PHI without your authorization (for example, as required by law, for public health activities, or in cases involving abuse or neglect);
- Your rights regarding your PHI, including the right to request access to and receive a copy of your health record; request an amendment to your health record; request an accounting of certain disclosures we have made; request restrictions on certain uses or disclosures; request confidential communications by an alternative means or location; receive a paper copy of our Notice of Privacy Practices at any time, even if you agreed to receive it electronically; and file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights, if you believe your privacy rights have been violated.
You may request a copy of our full Notice of Privacy Practices by contacting our office at (215) 918-7830 or info@humanitashealthservices.com. We will not condition treatment, payment, enrollment, or eligibility for benefits on whether you agree to a use or disclosure of PHI, except where permitted by law.
Secure Communication
Standard email and web forms are not fully secure methods of transmitting health information. To help protect your privacy, we ask that you:
- Avoid including PHI, such as diagnosis, medications, or insurance details, in messages sent through our website contact forms or by unencrypted email;
- Call our office directly at (215) 918-7830 for any communication involving a patient’s specific medical information;
- Use any secure patient portal or communication tool your Humanitas care team has provided directly to you for care-related messages, rather than general email.
If you choose to email us with information related to your care, please understand that standard email is not encrypted end-to-end unless we have specifically provided you with a secure means of communication, and that we cannot guarantee the security of information transmitted this way.
