What is Hospice Care?

Hospice care is provided in patients’ homes or residential and care settings (Skilled Nursing and Long-Term Care facilities, Assisted and Independent Living facilities) by interdisciplinary teams of professionals and trained volunteers. These teams ease the physical, emotional, and spiritual pain and stress experienced by patients and their families. Additionally, we offer thirteen months of grief support for families. Hospice care is available during the last six months of life through the Medicare Hospice Benefit.

Your care team collaborates with you, your doctors, and caregivers to address your overall well-being to create and facilitate a personalized plan of care.  They focus on helping you live and pass on with comfort, peace, and dignity, while also preparing and supporting your family members to honor your wishes and begin healing.  Humanitas Hospice is committed to integrating seamlessly with the patient, their environment, and those caring for them.   Our patients and their families are at the heart of everything we do.  Caring for you in your last stages are life is an honor.

Our Services Include

  • Comprehensive Care Team:

    Expert hospice and palliative medical and clinical professionals provide pain and symptom relief, emotional, spiritual, social, and bereavement support, caregiving training, and healthcare decision guidance.  Our team is available 24/7.

  • Specialized Therapies to Enhance Quality of Life:

    Physical and Speech Therapies are available for patients requiring extra support to ensure patient and caregiver safety.  Music Therapy is available to all Humanitas patients to promote quality of life and relaxation.

  • Durable Medical Equipment:

    The medical equipment you need is provided by Humanitas at no cost to you.  This equipment can include walkers, wheelchairs, hospital beds, specialized mattresses, oxygen, and more.  Deliveries can be made in as little as 2 hours to private residences or facilities.

  • Pharmaceuticals:

    All medications related to your Hospice diagnosis ae covered under the Hospice benefit without deductibles or co-pays.

  • Grief Support:

    Grief and Bereavement counseling starts on the day of admission and follows the family and/or caregivers for up to 13 months after the patient passes.

  • Caregiver Training:

    Taking care of an individual with increasing needs at the end of life is difficult, and often a new experience for many.  Our expert team will train caregivers on best practices to keep their patient or loved one comfortable, at peace, and safe.
