What is Hospice Care?
Hospice care is provided in patients’ homes or residential and care settings (Skilled Nursing and Long-Term Care facilities, Assisted and Independent Living facilities) by interdisciplinary teams of professionals and trained volunteers. These teams ease the physical, emotional, and spiritual pain and stress experienced by patients and their families. Additionally, we offer thirteen months of grief support for families. Hospice care is available during the last six months of life through the Medicare Hospice Benefit.
